About This Service
Supporting Older Australians to Live Well at Home
As we age, our nutritional needs become more specific and meeting them plays a major role in protecting strength, mobility, energy, and emotional wellbeing.
My Commitment to Care
We believe every person deserves access to high-quality nutrition support, without the stress of travel or waitlists.
That’s why we bring expert dietitians, practical guidance, and compassionate care directly to your home or aged care community.
Our goal is simple: to support healthy ageing with confidence, dignity, and tailored nutrition advice.
Common Areas of Support
How Dietitians Support Healthy Ageing
- Preventing and Treating Malnutrition Malnutrition is common in older adults and increases the risk of frailty, falls, infections, and hospitalisation. Our dietitians assess risk and create personalised plans that rebuild strength and wellbeing.
- Managing Chronic Health Conditions We support clients living with diabetes, heart disease, osteoporosis, gastrointestinal conditions, and more, creating nutrition plans that complement medical care and reduce complications.
- Support for Dementia & Swallowing Difficulties Reduced appetite, dysphagia, and memory changes can make eating difficult. We adapt nutrition and offer expert guidance on IDDSI texture-modified diets to support safe, enjoyable eating.
- Maintaining Independence & Vitality Good nutrition helps maintain muscle mass, cognition, and energy levels, reducing falls risk and improving quality of life.
- Holistic, Person-Centred Care We work closely with GPs, nurses, speech pathologists, carers, and allied health professionals to ensure nutrition care is integrated and aligned with your broader health plan.
- Navigating the Aged Care System Our dietitians are familiar with government programs like Home Care Packages, CHSP, and Meals on Wheels, helping you access practical support with confidence.

Why Nutrition Matters as We Age
Good nutrition is one of the strongest predictors of healthy ageing. Staying well nourished supports immunity, strength, mobility, mood, and independence. In Australia’s ageing population, dietitians play a vital role in ensuring older adults don’t just live longer, but live better.
Our Nutrition Support At Home Program bridges the gap between clinical care and everyday living, helping older Australians feel supported, understood, and nourished.

How It Works
01
Flexible, Accessible Dietitian Support
We offer both in-home and in-clinic appointments (Buderim). We can visit your home, aged care residence, or community setting.
02
What to Expect
Your dietitian will complete a full nutritional assessment, taking into account:
- Medical history
- Medications
- Weight and health changes
- Biochemistry
- Dietary intake
- Appetite and eating behaviours
- Personal preferences and lifestyle
- Clinical requirements (IDDSI, allergies, chronic conditions)
From there, we design a tailored nutrition plan to support your health, wellbeing, and independence.
03
Ongoing Support
- Weight monitoring
- Supplement reviews
- Regular check-ins
- Adjustments to your nutrition plan as your needs change
